Before we can schedule your appointment, you need to complete the steps outlined in the email (also summarized below).Ā
- Fill out your intakeĀ questionnaire.
- Log in to our electronic medical record to complete yourĀ consents/legal forms and then afterwards to retrieve your lab requisition.
- After you have retrieved your lab requisition go get your blood drawn at Labcorp or whatever is specified on the requisition. We recommend scheduling an appointment at Labcorp.Ā
- Once we receive the lab results then we will contact you to schedule your foundational assessment appointment.Ā
